One of the primary concerns raised in the petition was the mismanagement of the examination process. Reports surfaced about technical issues, delays, and inconsistencies in how the exam was conducted across different centers. Some students also alleged that the exam paper was leaked in certain regions, giving an unfair advantage to a specific group of candidates. The 2024 MDCAT exam held in Sindh, specifically organized by Dow University of Health Sciences (DUHS), is under significant scrutiny due to allegations of a paper leak. Reports claim that the MDCAT paper was leaked on social media before the test, with some students asserting that over 160 out of 200 questions matched the leaked content. This has led to protests by students in Karachi and other parts of Sindh, demanding the exam be annulled and re-conducted.

The Sindh High Court has been approached with a petition to investigate the incident, with calls for a Joint Investigation Team (JIT) to be formed to look into the matter.
The MDCAT conducted by Shaheed Zulfiqar Ali Bhutto Medical University (SZABMU) has faced multiple controversies and issues recently. Several students filed petitions, claiming that their exam contained numerous out-of-syllabus questions. Some students reported that more than 20 questions did not align with the syllabus, and even the provided answer key contained errors. This led to widespread dissatisfaction among test-takers.
In response to these grievances, the Islamabad High Court (IHC) initially barred SZABMU from releasing the results and ordered further investigations into the matter. Students demanded a retake of the exam, citing unfair advantages for students from other regions where the exam was easier.
